Since we have these passionately held beliefs, we feel bound to communicate to our residents and their families what progress is being made. We do this through our LifeCare Plans and Monthly Assessments. The Goal of LifeCare is to enhance quality of life. We assess each resident monthly and give the results to them and their families.
 
     

We call this assessment LifeCaresm.This tool encompasses not only current personal care needs (bathing, dressing, eating, incontinency, etc) but also dietary, physical fitness and socialization goals. This is very time consuming, but we have found that it ensures that every resident is closely reviewed on a consistent basis. The LifeCaresm “scorecard” can be compared to prior months so that we can see if any trends are developing.

Our Residents Deserve Our Best

  Our goal is to develop a care plan that is specific to each individual and encompasses the "whole" person. (Seasons are not a people warehouse.) We believe that each resident, no matter what their physical or mental capacity, still deserves to be cared for as a unique human being...with services that meet their many needs, an understanding that they are living life as best they can, and a deep respect for the special person who has come to live with us.

 
   
 
Here's How We Do It

  Developing and implementing 'Personal' Care plans takes a team effort. Here is how we build teamwork into the care we provide at Seasons:
  • Ask for the family's input on habits, likes/dislikes, abilities, etc.
  • Review the medical information with our nurse and design the care management and medication management plans.
  • Review the individual needs with our caregivers and organize the daily, personal care.
  • If the resident requires hospice care, we meet with the hospice team leader and review their plan.
  • Review the resident's interests and abilities with our Activity Director and develop an activity plan.
  • Review the dietary information with our chef and create a dietary plan.
   
         
   
Coordinated Health Care

  At Seasons we make access to healthcare easy. In many of our communities, we have a health-care clinic, where physicians come into the building and see our residents.

Every effort is made to bring the personal physician of the resident into the community on routine basis. The physician works closely with our licensed nurse and our care staff.  If a resident experiences an , such as a urinary or respiratory tract infection, we can get home health nurses on-site 24 hours a day to assess the resident and communicate timely with their physician. It is infrequent that a resident has to be transferred out to a hospital for medical evaluation since our nurses are usually able to evaluate symptoms promptly and obtain medical treatment with the first sign of illness.
